Inputs, controls, outputs and evidence
A quality system is not a collection of forms. Each process transforms inputs into outputs for a customer or the next process. Define the owner, criteria, resources, risks, controls, records and measures that show whether the process works.
Documentation should be proportionate to risk and complexity. A record proves what happened; a procedure explains how work should happen. Copying a signature without verifying the activity creates paperwork, not assurance.
